Grrr...I've been chatting with Amazon for an hour and they are telling me the Matchbook price is $8.00.

I bought it from Amazon.com. The paperback version isn't available on Amazon.ca.
There shouldn't be a problem then. If you're chatting with Amazon, just tell them to pull up the Kindle page: http://www.easywdw.com/reports13/kindlematch.jpg It says right on it that it's free with a print copy.
Okay, after another 40 mins on the phone with Amazon, they said that Matchbook is for US customers only. The customer service rep didn't know how others outside of the US were able to get it.
Like others on the site, they gave me a credit for the Kindle version, so there is a happy ending to my saga!
I'm happy to now have my Kindle version and I look forward to the paperback version that should be delivered by Thursday.

That's too bad, their online chat is usually pretty lenient on refunds and credits. Unfortunately, Amazon's policies are almost entirely out of my hands and they're a lot better at some things than others.
I'm just happy I was able to get both. They did the right thing in the end!
